Health Declaration & Liability Waiver

By booking or attending any class at SPARK Studio, I confirm the following:

Health & Fitness

I confirm that I am physically fit to participate in Pilates and group fitness classes. I will disclose any injury, pregnancy, medical condition, or physical limitation to my coach before each class, and I understand that SPARK Studio may refuse or modify my participation for safety reasons. I understand that class levels are assigned based on experience and fitness, and advanced classes may require coach approval.

Assumption of Risk

I understand that participation in physical activity carries inherent risk of injury. I voluntarily assume this risk and agree to follow all coach instructions and use equipment correctly, including wearing grip socks for Reformer/Microformer classes.

Release of Liability

I release SPARK Studio, its coaches, and staff from liability for any injury, loss, or damage arising from my participation, except where caused by proven gross negligence or willful misconduct. I understand SPARK Studio is not responsible for personal belongings left in lockers or changing rooms.

Booking, Cancellation & No-Shows

I understand that bookings are made through the SPARK Studio app, and that:

  • Cancellations must be made at least 8 hours before class to retain my credit.
  • Late cancellations or no-shows result in loss of the class credit.
  • Repeated no-shows/late cancellations may restrict my booking access.
  • Packages are personal, non-transferable, non-refundable, and expire per their validity period.

Studio Conduct

I agree to arrive on time, wear appropriate attire, and behave respectfully toward staff and other members. I understand disruptive, abusive, or unsafe conduct may result in removal from class or termination of my membership without refund. I will not photograph or film in the studio without prior approval.

Minors

If I am booking for a minor, I confirm I am their parent/legal guardian and consent to their participation under SPARK Studio's supervision requirements.

Data & Privacy

I consent to SPARK Studio collecting and storing my health and personal information for the purpose of ensuring safe participation, in accordance with applicable data protection laws. I understand I may opt out of marketing communications at any time.

Acceptance

I confirm that I have read, understood, and agree to this waiver and to SPARK Studio's full Terms and Conditions, as may be updated from time to time and communicated via the app.
